Malaysia : Attachment

Bismillahirrahmanirrahim…

Assalamualaikum wbt… selawat & salam ke atas junjugan besar Rasulullah saw.. Alhamdulillah atas nikmatNya kita masih menarik nafas di bumiNya.

Atas nikmatNya juga saya berpeluang pulang ke Malaysia setelah lebih kurang 2 tahun di perantauan…Atas nikmatNya juga saya kembali berpijak di bumi kinanah Mesir bagi menyambung pengajian di sini…

Post kali ini saya nak berkongsi pengalaman saya menjalani attachment/ elective posting di Malaysia… Tempat: Hospital Tuanku Fauziah, Kangar.. Department : Paediatrics, Emergency & Medical… Duration : 2 weeks.. Elective posting ini terpaksa saya jalani pd bulan Ramadhan, kerana tiada waktu lain yg mengizinkan… Nawwar , syarifah (Cairo) dan Zahrah (Indonesia) menjadi teman saya di hospital.

Hari pertama mak saya hantar dan ambil.. tapi untuk hari2 berikutnya saya drive sendiri ke hospital🙂 (perjalanan ke hospital cuma 5-10 minit) Mungkin sesetengah org rasa xde pape drive..😉 tapi sebenarnya ini kali pertama saya drive tanpa abah saya teman.. dulu saya penakut, padahal lesen saya ambil sejak lepas SPM lagi…thn ni baru betul2 bawak kereta…itupun saya akan keluar lepas pukul 8 pagi (nak elakkan jam) dan parking jauh sket (spy sng nak keluar)..maklumlah, x cekap sgt… eventhough mcm tu…:) saya rasa bersyukur dan berbangga jugak kot dgn diri saya yg dah berani lwn rasa penakut bawa kereta tu… sampai bila pun xkan cekap kalau x bawak sendiri…  ;)…

Paediatrics dept

My 1st 4 days di Paeds dept…Dr Jamal , Dr Nasir , n Dr Lim ialah dr2 pakar di situ yg bnyk mengajar saya, selain dr2 MO dan HO yang lain… And surprisingly, ada 2 org HO di situ yg baru grad from Cairo Uni… so diorg bnyk juga share pglmn jadi HO… Kata diorg jangan risau la , even kita (Egypt’s grads) mcm kurang sket practical…itu semua boleh belajar time HO… terus-trg je dgn dr kalau ada ape2 yg kita x tahu or x pernah buat…but at the same time show ur enthusiasm n willingness to learn…kata diorg lagi, biasa la kena marah n so on…that’s the part n parcel of life… nama pun HO…

Tapi saya rasa, HOs n elective students mcm saya ni bertuah kerja or attach di GH(general hosp)…sbb dr2 mostly baik2 belaka…compared to teaching hospital (university hospital), dr2 situ dah biasa dgn students, so dia mcm garang sket kot..:) (atleast my few friends and i think so)

Apart from ward round, i also got the chance to join these HO in their CME session..CME stands for Continous Medical Education..mcm seminar, prepared by one HO, attended by other HOs and evaluated by Senior doctors.. everybody (the presenter n the audience) will be evaluated… [feel like assignment kat sini pon ada]… and on the day of the CME, as I was there, so i’m also included in the Q&A session…cuak juga , sbb setiap org kna prepare satu soalan.. Mmg la tajuk IRDS je pon…tapi IRDS itu tahap dr2 dah… nama ubat n dose2 sume… berpeluh saya merekacipta soalan…n at the same time, i was hoping that anybody could defend that i was just an elective student (in fact, not a clinical stdnt yet).. sampai turn, saya terpaksa tny juga soalan yg sy reka tu…then only the doctor realized that i was just a stdnt… anyhow, he responded quite well…katanya…”its good enough for a 3rd year student to be able to ask this kind of Q” heheeh…lega…

Emergency dept

My next dept was the A&E dept… situated at the new building..so cantik sket la… the day nawwar n i had to register, nawwar had to bring her sis to the dentist, so i myself went there to register… i met with the head of A&E dept, Dr Ida Hazliza… i was quite nervous , but her warm descent smile and greeting felt very welcoming to me… n guess what’s next…

Dr Ida (aka the head of dept) sendiri yg bawa saya jalan2 dlm emergency dept tu, kenalkan pada setiap unit, kenalkn pada HOs MOs, paramedics n the nurses…”perkenalkan, ni ada stdnt dari Mesir ,Nurul Nazihah Mohd Nor akan buat elective posting kat sini..harap semua dpt bantu dia” … sounds very welcoming right…

A&E consists of many subunits(subareas)… beginning with the operator..comprising  the public connection, the big, precise map of Perlis n Northern Kedah, and the special connection whre the operators can communicate directly with policeman and firemen team…

On the other section, after reception, vital signs of patients will be recorded, before proceeding to the next door. then, the patient will proceed to the semi critical zone and the process of stabilizing the patient is performed.. lab and radio xm will be ordered for these patients and while waiting for the result, they’ll be placed in the temporary wards which is still inside the same A&E unit…after the result is ready, the patient will either be admitted to ward or discharged.

For highly emergent condition, the patients will directly enter the critical/ red zone..the alarm will be ringed, so that all the doctors will rush there to resuscitate the patient..

Actually the best dept i practiced is this A&E dept…well, once i told her that we were a bit lacking these practical works (like setting lines, blood withdrawal, suture n etc) she assured me that i can do them all here… “Kami akan pastikan awak boleh buat prcedure2 ni dlm masa 4 hari awak di sini” hehhee….Selain tu, di Emergency, kita akan dpt variety of case… mcm2, emergency hypertension, cardiac, accident, stroke, abortion, snake venom, appendicitis, fainting, n etc…medicolegal cases pun ada, kes gaduh, kes rogol pun ada…bulan2 puasa pon ada..

Among procedures yg kami sempat buat ialah line setting and blood withdrawal… yg lain (suture, male n female CBD-catheter , nasogastric tubing n so on) sempt blajar tgk je, x sempat buat…

  3. assalamualaikum, saya tengah menggoogle pasal cara-cara nak buat attachement bagi pelajar malaysia yang study luar negara, dan terumpa blog ni. Boleh tak kalau tak keberatan share-kan cara macammana nak apply, in term of borang and etc?

  4. Afiq: Waalaikumsalam wbt.. Borang bole download drp minstry of health rasanya, send online pon boleh iA. Kalau dulu, tak strict sgt, boleh buat 2 weeks or less… now, kalau nak buat mesti 1 month kot, not sure.
    yg pntg, kena ada
    1- surat uni, approve kita thn berapa , uni mana, nak buat attchmnt kat hosp mana.
    2- borang attachment [download fr MOH]
    3- formal letter kpd pgrh hosp, apply dept mana, tarikh bila etc. [dulu , skg xtahu kena ke x]

    kena apply atleast 3months from date of attchment.
    Thats all i can giv.. harap membantu.
